Checklist for Kindergarten Registration

-Complete the pre-registration using this link:  https://provo.edu/kindergarten-registration/ -Once you have completed the pre-registration, look for an email from the school. You will be directed to complete the second part of the online registration. -Bring in to the office your photo ID to prove that you are the child’s parent, your child’s original birth certificate, immunization records and proof of residency. If you don’t live in the school’s boundary, please complete an open enrollment form using this link:  https://provoedu.ca1.qualtrics.com/jfe/form/SV_bxXLLozRCwTsmjP
